Western Isles Health And Social Care Partnership

JPIAF Submission May 2006/07

Introduction

The JPIAF evaluation is contributing to the developing integrated approach being taken forward by the Western Isles Partnership.

We invited the Joint Improvement Team to work with us in June 2005 to help deliver service improvements through improved partnership working and more effective use of resources. The work programme has focused on:

The JIT is also providing support to the management of delayed discharge in context of whole system working.

We believe that the Partnership is making good progress in taking forward these objectives. The aim to have an integrated Community Health and Social Care Partnership is one we still aspire to and will continue to focus on the development of integrated financial and HR protocols to support this. The Health and Social Care Partnership Project Team was established in 2005 to oversee and manage the Partnership programme, reporting to the Joint Future Committee. We are developing more robust performance arrangements to support our work and the JPIAF provides a useful framework for this.

We recognise the particular challenge of delivering services to the WI population and by working more effectively together we believe we can deliver high quality services building on some excellent local examples. The whole systems approach is one we are signed up to and we are keen to develop the JPIAF approach across all community care groups. We have made efforts to include specific LITs for people with learning disabilities, mental health problems and drug and alcohol misuse and will build on these over the next few years.

We continue to work to better engage with people who use services and family carers. Locality groups are in place or being established, and over time we would like to develop specific locality LITs relevant to the needs of the different communities.
