Joint Performance Information and Assessment Framework (JPIAF) Submission May 2007

Introduction

The Western Isles Health and Social Care Partnership is pleased to submit the JPIAF 2007 evaluation.  This early submission is a reflection of the progress that has been made in the past year and of the high profile joint working has achieved to enable better outcomes for the people who use Health and Social Care services in the Western Isles.

The continued support of the Joint Improvement Team over the past year has assisted the Partnership in ensuring good progress in taking forward objectives identified in the  work programme established in June 2005 when JIT were invited to help us focus on improved partnership working. Copies of the JIT brief, work plan, progress reports and exit report are available.  We continue to benefit from their support as we now concentrate on specific issues such as Joint Planning and Commissioning.

The JPIAF evaluation allows us to reflect on significant milestones as we have progressed in our development of an integrated approach:

The Western Isles Community Health and Social Care Partnership

Last year the development of a Community Health and Social Care Partnership was an aspiration. This year we are pleased to report this has become a reality.  The long history of successful partnership working in the Western Isles has been formalised with the establishment of the CHaSCP (Community Health and Social Care Partnership) and this will continue to develop the Joint Future Initiative for the delivery of services, community planning and enhance the current local arrangements.

Commitment to Joint Working

The commitment of Comhairle nan Eilean Siar, Western Isles Health Board and the Voluntary Organisations to joint working is evident in this years submission as we report an improvement in both National and Local Improvement Targets.  The Partnership is committed to making good progress with the joint management of delayed discharges, developing integrated locality based services such as the Carloway Care Centre, Lewis and Taigh a Chridhe Uile Naomh., South Uist and also in jointly addressing and managing service pressures.

Changing Traditional Behaviours

We have observed and experienced positive changes in traditional behaviours as we develop an effective strategic performance Management Framework and Implementation Programme to take account of NHS and Local Authority approaches, supported by an organisational development framework.  The Health and Social Care Project Team established in 2005 with Health, Local Authority, Voluntary Sector and User representation will become the management team to oversee and manage the development of the CHaSCP.

Protocols, centred around the use of the single shared assessment have been developed to allow access to resources across agencies and these are now being implemented breaking down traditional professional barriers and ensuring a seamless service at the point of delivery.

Centred on Better Outcomes

The Western Isles Partnership recognises the need for effective joint working to ensure the delivery of high quality care.  The Community Health and Social Care Partnership is committed to the better outcomes approach and we will maintain a high level of intensive Home Care provision to ensure individuals are assisted to remain at home where-ever appropriate.  We continue to monitor the use of the SSA and Carers Assessments as well as the time intervals between completion of assessment and delivery of services.

Although not reporting on new LITs at this point, the Partnership has set new targets which will be monitored and reported to the Health and Social Care Group on a quarterly basis.  We welcome the New Outcome Measures and we have included these in our new targets. For example, as a way of measuring quality of services user safety and satisfaction will be sought.  Engaging with as many relevant stakeholders in the JPIAF process has proved beneficial and ensures better understanding of the processes as well as highlighting gaps in collection of data.

Continuing to Make Progress

As we look forward to feedback and evaluation of our JPIAF submission we continue to strive to make progress to enhance the health and social care provision for the people we serve to ensure a healthy and happy population here in the Western Isles.
